Medical schools have also witnessed another important change there has been a steady increase in female enrolment. The data, provided in CIHI's health provider report and attributed to Canadian Medical Education Services and the Association of Canadian Medical Colleges, show that female enrolment has exceeded male enrolment since 1993 94. Physician Distribution Issues Canada not only has a supply problem, but there is a well-documented problem of distribution as well. CIHI's health provider report30 noted that there is an average of 94 family physicians per 100, 000 Canadians in some health regions in Canada. However, some health regions have rates over 140: 100, 000, while others have a ratio of less than half the national average. Governments have tried to address this problem in a number of ways. New Brunswick has attempted to manipulate where physicians can practise by controlling billing numbers. Quebec has discounted the fees new doctors can charge if they choose to set up shop in overserved areas. Others have tried the "carrot" versus the "stick" approach. Return-service agreements are becoming increasingly popular: contributions to education costs in exchange for a commitment to practise in a rural or remote area upon graduation. Provinces have also traditionally relied on foreign-trained physicians to fill their needs in rural and remote areas. In its submission to the Commission on the Future of Health Care in Canada, ACMC pointed out that well over half of physicians entering medical practice in Canada in the 1960s were international medical graduates IMGs ; 31. However, in 1975, physicians entering Canada no longer had "preferred status, " and the numbers of IMGs entering Canada has dropped from 1, 090 in 1974 to 245 in 1997. Currently, IMGs make up less than a quarter of the physician workforce. Interestingly, the proportion is only slightly higher 26% ; in rural Canada. A renewed effort to recruit physicians from other countries is currently underway. Ontario, for example, is doubling the number of foreign physicians assessed and trained to work in the province. Manitoba has a similar program, with the intent that IMGs who pass a qualifying exam are granted a conditional licence working with another licensed physician who acts as a "practice advisor." However, a perennial problem remains. Foreign-educated physicians have generally not done as well as Canadian-educated physicians in licensing exams. ACMC points out that the success rate of IMGs in Medical Council of Canada qualifying exams is approximately 21% compared to 95% for Canadian medical graduates.

Based on this observation, we designed a series of experiments to characterize the development of enhanced, non-apomorphine-induced erections or 'spontaneous' erectile responses to vehicle administration in rats with previous exposure to APO. We further examined the effects of castration on these conditioned erections. Naive ie never received APO ; rats were administered vehicle 1 ml kg saline ; to determine the frequency of baseline erections and yawns. An alternating series of APO 80 microg kg s.c. ; and vehicle administrations were performed over several days and subsequent erectile and yawning responses were recorded. Following 3 sets of 3 APO administrations with vehicle administered between sets ; , and the 3rd vehicle administration, these rats were then surgically castrated and allowed 30 days to recover. Following this, APO was administered 3 times to determine erectile and yawning responses post-castration, followed by vehicle administration to determine the effects of castration on conditioned APO responses. The major findings were: 1 ; that although naive rats had a basal spontaneous erectile response 0.75 + - 0.88; 4 of 8 rats with at least one erection ; , repetitive administration up to 22 treatments ; of the central initiator apomorphine significantly increased the number of erections 1.8 + - 0.7; of 8 rats with at least one erection ; and yawning 2.5 + - 2.47 ; responses to vehicle administration; and 2 ; both spontaneous yawning and erectile responses were found to be androgen dependent since castration dramatically lowered the number of erections 0.13 + - 0.35; 1 of 8 rats with at least one erection ; and yawns 0 ; . The present study investigates whether the mechanism of attentional modulation of motor performance is normal in patients with Parkinson's disease. Neuropsychometric testing of patients has suggested impairments on tests sensitive to frontal lobe damage Lange et al., 1993; Stam et al., 1993; Owen et al., 1997 ; , proportional to the reduction in frontal and striatal dopamine metabolism Rinne et al., 2000 ; . However, the abnormalities in prefrontal function in patients with Parkinson's disease may result from abnormal interactions between areas, rather than abnormalities within areas. It is this possibility that is studied in the present paper using functional brain imaging. The motor programming decits of Parkinson's disease have been proposed to result from a functional disconnection of the supplementary motor area SMA ; Dick et al., 1986 ; . In addition, effective treatment of Parkinson's disease by pallidotomy has been associated with changes in cortical and subcortical connectivity of the SMA Grafton et al., 1995 ; . Functional imaging of patients during simple movements has yielded a complex pattern of results. A recurrent nding has been reduced activity of the SMA, as measured by PET Jenkins et al., 1992; Playford et al., 1992; Rascol et al., 1992; Samuel et al., 1997a; Thobois et al., 2000 ; , functional MRI fMRI ; Sabatini et al., 2000; Haslinger et al., 2001 ; and event-related potential ERP ; Jahanshahi et al., 1995 ; , especially when the movement or its timing is chosen by the subjects themselves Cunnington et al., 1995; Jahanshahi et al., 1995; Praamstra et al., 1996; Catalan et al., 1999 ; . Furthermore, this underactivity has been reversed by treatments including L-dopa Haslinger et al., 2001 ; , apomorphine Jenkins et al., 1992; Rascol et al., 1992 ; , pallidotomy Grafton et al., 1995; Samuel et al., 1997b ; and stimulation of the subthalamic nucleus Ceballos-Baumann et al., 1999 ; . However, impaired activation of the SMA is not a consistent nding. Catalan et al. 1999 ; showed that increasing complexity of a learned motor sequence task led to greater increases in activation of the SMA in patients than in controls. Sabatini et al. 2000 ; reported increased activity of the caudal SMA in patients during sequential nger movements, and Samuel et al. 2001 ; observed no difference in SMA activation in a task that required subjects to select freely between movements and hold the selected moves in memory. Many of the studies that have shown SMA impairment with parkinsonism used motor tasks requiring a degree of attention to action, including free selection of movement or freely selected timing of movement. In contrast, movements that were specied by an external cue, or by their place in a xed overlearned sequence, were associated with lesser or no decits in SMA activity Cunnington et al., 1995; Jahanshahi et al., 1995; Praamstra et al., 1996; Catalan et al., 1999; Nakamura et al., 2001 ; . It is therefore necessary to distinguish and apomorphine.

Although no positive Turnbull blue reaction was observed in female gametes, transmission electron micrographs revealed dispersed ferritin clusters within the cytoplasm of oocytes to which ferritin-containing hemocytes follicle cells were appressed Fig. 5.7, 5.8 ; . The localization and distribution of ferritin within the various cell types involved in the transfer sequence presented notable differences, which also explains the absence of a visibly positive Turnbull reaction in some cell types. Both the intestinal cells and the transport hemocytes possessed variously sized inclusions with considerable concentrations of ferritin, and these cells presented visibly positive Turnbull reactions. Upon histological examination, 3 of the 12 experimental scallops were observed to be mature and ready to spawn, while the remaining 9 had already spawned and had begun producing a new cohort of female gametes. Although ferritin appeared in the intestinal epithelium and surrounding connective tissue of the mature scallops, ferritin-containing hemocytes were virtually never observed, either in the acini, or appressed to the oocytes of these individuals. Moreover, despite the presence of mature residual oocytes in the acini of the individuals which had spawned previously, ferritincontaining hemocytes follicle cells were never observed appressed to them. Ferritin-containing hemocytes follicle cells were, however, observed appressed to late-developing residual oocytes in these individuals Fig. 4.8 ; .
