Ephedrine is a sympathomimetic amine that is found naturally in Ma Huang, Sida Cordifolia, and about 40 other species of plants. Ephedrine is not a "selective stimulant", so it will exert stimulating effects on all tissue which has receptors for it. This includes the nervous system and cardiovascular system. The actions of ephedrine that are desired by athletes include increased alertness, "energy boost", assistance in weight loss, and the all-inclusive enhancement of athletic performance. Since ephedrine is a stimulant, it will combat fatigue and allow for improvement of performance when an individual is in a fatigued state. When one is not fatigued, the effects are not known. There are adverse effects associated with its use. The two main systems affected by ephedrine are the cardiovascular and neurologic. The cardiovascular effects include elevated blood pressure, cardiac arrhythmias, strokes, and myocardial infarctions. Neurologically, the effects include seizures, tremors, insomnia, irritability, and headaches. There have been a number of deaths reported that were temporally related to the use of ephedrine. Another problem with all stimulants is dependence. When a person regularly uses a stimulant, the normal state becomes the stimulated state. When the stimulant is not taken, the person does not feel "right" or "normal". Caffeine and the daily cup of coffee is the best example of this dependence. WADA USADA, NFL, and NCAA ban ephedrine. The FDA has recently banned ephedrine. That ban will take effect this year 2004.

FULL REPORT I. 1. QUESTIONS What is the role of capecitabine as second-, third-, or fourth-line chemotherapy in stage IV metastatic ; breast cancer? What is its role in anthracycline failure? What is its role in taxane failure? 2. What is the role of capecitabine as first-line chemotherapy in stage IV metastatic ; breast cancer? II. CHOICE OF TOPIC AND RATIONALE Capecitabine XelodaTM ; is a new and relatively expensive drug that has been approved by the Canadian Health Protection Branch for the treatment of patients with advanced or metastatic breast cancer after failure of standard therapy including a taxane, unless therapy with a taxane is clinically contraindicated ; . It is the first oral chemotherapy option approved for patients with advanced or metastatic breast cancer who do not respond to standard chemotherapy or for whom standard chemotherapy has failed. A critical examination of the clinical evidence is required to help inform decisions by clinicians and policy makers in Ontario. Capecitabine is a novel, selectively tumour-activated fluropyrimidine carbamate. The parent compound is inactive; however, after gastrointestinal absorption, capecitabine is hydrolyzed in the liver by carboxylesterase to produce 5'-deoxy-5-fluorocytidine. This moiety is then deanimated on its pyrimidine ring to produce 5'-deoxy-5-fluorouridine 5'-DFUR ; by cytidine deaminase, an enzyme primarily located in hepatic and neoplastic tissue. The final enzymatic step, selective tumour activation of 5'-DFUR to 5-fluorouracil FU ; , is catalyzed by thymidine phosphorylase, an enzyme typically found in higher concentrations in solid tumours than in corresponding normal tissue. This phenomenon, coupled with the unique activation pathway of capecitabine, results in intra-tumoural concentrations of 5-FU that are significantly higher than plasma and normal tissue levels. Thus, it is hypothesized that the tumour-selective generation of 5-FU with low systemic exposure provides an improved therapeutic ratio for capecitabine. III. METHODS This guideline report was developed by the Practice Guidelines Initiative PGI ; of Cancer Care Ontario's Program in Evidence-based Care, using the methods of the Practice Guidelines Development Cycle 1 ; . Evidence was selected and reviewed by two members of the PGI's Breast Cancer Disease Site Group DSG ; . Members of the Breast Cancer DSG disclosed potential conflict of interest information. The guideline is a convenient and up-to-date source of the best available evidence on the use of capecitabine in patients with metastatic breast cancer, developed through systematic reviews, evidence synthesis, and input from practitioners in Ontario. The body of evidence in this report is primarily comprised of mature randomized controlled trial data; therefore, recommendations by the DSG are offered. It is intended to promote evidence-based practice. The PGI is editorially independent of Cancer Care Ontario and the Ontario Ministry of Health and Long-term Care. External review by Ontario practitioners was obtained through a mailed survey consisting of items that address the quality of the draft practice guideline report and recommendations and whether the recommendations should serve as a practice guideline. Final approval of the guideline report will be obtained from the Practice Guidelines Coordinating Committee PGCC ; . The PGI has a formal standardized process to ensure the currency of each guideline report. This consists of periodic review and evaluation of the scientific literature and, where appropriate, integration of this literature with the original guideline information. An economic evaluation was undertaken to compare the cost-effectiveness of UFT LV and capecitabine with that of intravenous 5-FU LV. Intravenous 5-FU LV is an appropriate comparator because it is the most common firstline treatment for metastatic colorectal cancer currently in use in the UK. A number of intravenous 5-FU regimens are in common use in the UK: the Mayo regimen, the de Gramont inpatient and outpatient regimens ; , the modified de Gramont MdG ; regimen and continuous infusion regimens. These are detailed in Appendix 2. The decision as to which regimen to use depends on the preferences of the physician and the patient, the resources available at the local treatment centre and the distances the patient may have to travel in order to receive treatment.

Guideline Question What is the role of raltitrexed Tomudex ; in the management of metastatic colorectal cancer? Target Population These recommendations apply to adult patients with metastatic colorectal cancer for whom chemotherapy is indicated. Recommendations For patients with previously untreated metastatic colorectal cancer in whom chemotherapy is indicated, a combination of 5-fluorouracil 5-FU ; plus leucovorin LV ; and irinotecan is now the standard treatment regimen. For patients with previously untreated metastatic colorectal cancer where monotherapy with fluoropyrimidines or other thymidylate synthase inhibitors e.g. 5FU LV or capecitabine ; appears appropriate, it is reasonable to offer raltitrexed as a therapeutic option. Suitable patients would include those for whom toxicity from 5FU is a concern such as patients who are over 60 years in age or women ; , or for whom the more convenient administration schedule of raltitrexed is important one visit for raltitrexed every three weeks versus five daily visits with 5-FU every four weeks ; . At this time, there is insufficient evidence to make a recommendation for or against the use of raltitrexed in patients who progress on 5-FU LV.

Eight disintegrins were evaluated in this study. Table 1 lists the amino acid sequences for the compounds tested. Initially, small quantities of bitistatin, albolabrin, echistatin and eristostatin 7 ; were provided by Drs. McLane and Niewiarowski from the Thrombosis Research Center at Temple University. Subsequently, several additional disintegrins were isolated from crude venom in Dr. Knight's laboratory; bitistatin from Bitis arietans venom ; 8 ; , kistrin from Calloselasma rhodostoma ; 9 ; , mambin from Dendroaspis jamesoni ; 0 ; , halysin from Agkistrodon halys ; 11 ; and barbourin from Sistrurus milarus barbouri ; 12 ; . A general method was developed to separate each active disintegrin from other venom components. First, one gram of crude freeze-dried venom Miami Serpentarium Laboratories, Punta Gorda, FL ; was dissolved in 10-15 ml of 0.05 M ammonium acetate, pH 5.0, and then clarified by ultracentrifugation at 20, 000 rpm for 30 min. The supernatant was applied to a 2.6 X 25-cm column of CM-Sepharose Fast Flow Pharmacia, Uppsala, Swe den ; and eluted with a stepwise gradient of ammonium acetate solutions of increasing pH and ionic strength from 0.05 M, pH 5.0, to 1.0 M, pH 9.6 ; . The column was run at a flow rate of 15 ml min and fractions of 3 ml each were collected. Peaks were located by measuring absorbance at 280 nm of every fifth fraction. Peaks containing active disintegrins were identified by their ability to inhibit human platelet aggregation see below ; and then freezedried in shallow dishes and stored at " 70Cprior to further purification.
