In 1991, the first International Prader-Willi Syndrome Conference was held in the Netherlands and a few days later the International PWS Organisation, representing 22 national associations, officially formed and registered in Sweden. Our founders, parents and professionals, established this organization to represent all persons with PWS, their families, and all those who work and care for them throughout the world. Having just passed our 15th anniversary, following the "footsteps of the great souls that walked before us, " IPWSO is now the established global PWS ambassador, spreading education and following our mission "to raise the quality of life for all people with Prader-Willi Syndrome and their families. During this time, IPWSO has grown in size and in dimension of services, representing 76 member associations. Financed by subscriptions from member associations, by donations, and grants, IPWSO is a non-profit organization, run by volunteers, with one part time paid Director of Program Development and one part time Business and Technical Advisor, IPWSO operates with very little cash and an abundance of help from compassionate global friends. IPWSO fosters the formation of new associations throughout the world. To qualify as a member of IPWSO, a nation needs only one parent delegate and one professional delegate. Although this requirement sounds simple, in some countries, this is not a reality. One desperate mom wrote that she was going to kill herself because she could not find a single doctor in her country who knew anything about the syndrome. She had no hope for her newly diagnosed child, until we helped her find an interested physician. In one country, where the government does not allow Parent Associations, we have two professional delegates. Another association has a parent delegate who lives in another country. Our flexibility, to include all nations, also extends to the annual fee, based on membership, with a minimum fee of $ 50. Given the actuality of our emerging associations, this fee is often not attainable, so we also offer an associate membership, with no fee. IPWSO's heart extends to all people with PWS without discrimination. Education is the key to our objective of early diagnosis and early management of the syndrome. We strive to meet this goal by providing printed and electronic educational material in a multitude of languages to families and professionals. Global distribution of three educational packages General Awareness, Medical, and Crisis ; in a multitude of languages has been a significant accomplishment for IPWSO. Since less than half of our associations have publications, these precious packets represent the gateway to a better life for their children. In the past few years, we distributed over 6, 000 Educational packets in English and other languages. We dispersed an additional 5, 000 IPWSO Educational Packages in Spanish throughout Spain, Latin America, and the United States. Thanks to cooperative efforts with PWSA USA ; , in 2004 we produced a DVD Food, Behavior, and Beyond, " Pittsburgh Partnership ; . With collaboration and sponsorship from our USA association, we have distributed medical management books and hundreds of additional articles and publications. We are presently printing small Medical Alert booklets for emergency medical situations. IPWSO will distribute these to our associations and to physicians at major medical conferences. Many of our publications are also available by PDF, readily available for translation and printing globally IPWSO encourages initial national and continual regional conferences, providing support in programming and supplying speakers. Education extends also into lectures at medical schools and hospitals and for the past three years has included PWS Awareness Booths at major medical conferences, including regional Endocrinology meetings in Europe, Latin America, and Australasia. In 2005, IPWSO sponsored the first Prader-Will Syndrome training meeting for physicians at the Peking Union Medical College Hospital in China. Our website ipwso ; opens the doors to multilingual websites throughout the world. IPWSO has also offered a course on PWS group work for psychologists and we are now developing courses for scientists learning diagnostic techniques. One of the most important goals of IPWSO is to provide an international forum for scientists and multidisciplinary professionals. In addition to encouraging and facilitating a global network of research and services, IPWSO holds an international conference every three years. This conference in Cluj represents our "sixth international forum." Using our loud voice as one of the largest educational networks among rare disease organizations, we have formed cooperative projects and alliances with similar groups. Our final day of the International Conference 2007, with a day conference on Rare Disease, is an example.

Other mechanisms, particularly those for the synergy with docetaxel given at the middle during capecitabine 5'-dfurd administration, would also exist.
Bridgewater, NJ October 1, 2007 Sanofi-aventis announced that the U.S. Food and Drug Administration FDA ; has approved Taxotere docetaxel ; Injection Concentrate in combination with cisplatin and 5-fluorouracil for induction therapy of locally advanced squamous cell carcinoma of the head and neck SCCHN ; before patients undergo chemoradiotherapy and surgery. The FDA based its approval on the results of the phase III randomized, open-label, international trial, TAX 324, which established the efficacy and safety of the Taxotere-based regimen in significantly improving survival. Approval Based on Clinical Trial Tax 324 Among patients treated with Taxotere-based therapy TPF, n 251 ; overall survival was significantly improved compared to patients receiving just cisplatin and 5-fluorouracil PF, n 243 the relative risk of death was 30% lower HR 0.70; p 0.0058 ; . Patients treated with TPF had a longer median overall survival of 70.6 months vs. 30.1 months for patients receiving PF only, representing a more than three year improvement in median OS for patients treated with TPF. The probability to survive three years was 62% in the TPF arm compared to 48% in the PF arm. "The TAX 324 trial found that the addition of Taxotere to standard induction chemotherapy significantly improved patient survival, adding years to patients' lives, " noted clinical investigator Marshall Posner, MD, Medical Director of the Head and Neck Oncology Program at DanaFarber Cancer Institute in Boston. "The approval of Taxotere to be given in combination with other standard chemotherapy as the first step in a therapeutic sequence followed by chemoradiotherapy and surgery is a significant advancement in treatment for patients with locally advanced head and neck cancer." All patients entering TAX 324 had tumors of the oropharynx, larynx, hypopharynx or oral cavity that either could not be removed, were considered potentially operable but unlikely to be cured with surgery, or could not be removed in order to preserve organ function. Participants in the trial had either stage III or IV SCCHN with no distant metastases. Patients were treated every three weeks for three cycles with either TPF Taxotere 75 mg m2 plus cisplatin 100 mg m2 and 5-fluorouracil 1000 mg m2 a day for four days ; or PF intravenous.
The study population will be separated into two groups. The first group is patients relapsed after more than 6 months of adjuvant chemotherapy completion. Second group is primary metastatic patients who have failed to first-line chemotherapy and patients who have relapsed within 6 months of adjuvant chemotherapy as second-line chemotherapy. We suppose that fluorouracil derivatives are still effective agents for the first group patients. Because docetaxel seems to be effective in second-line or salvage setting, we will study docetaxel with other active agents such as irinotecan as a second-line or salvage therapy instead of repeated platinum exposure 25, 26 and docusate.

In the united states, iressa is indicated as a treatment for patients with locally advanced or metastatic nsclc after failure of both platinum-based and docetaxel chemotherapies.
Purpose: Bexarotene, a novel RXR agonist, is used in the treatment of cutaneous T cell lymphoma and is under investigation for the treatment of solid tumors in combination with other chemotherapeutic agents including docetaxel. Both docetaxel and bexarotene are metabolized by CYP3A4 and in animal studies bexarotene has been shown to enhance the activity of CYP3A enzymes. Thus, there exists a potential for interactions between these two agents, which was evaluated in this study. Method: Bexarotene from commercial capsules was dissolved in DMSO. Employing pooled human liver microsomes, we assessed the influence of bexarotene on the metabolism of docetaxel. Microsomal fractions 1mg protein ; were incubated with bexarotene 0.1 10M ; and docetaxel 0.5 5M ; in the presence of NADPH. Docetaxel levels were measured using a validated HPLC method. Next, we employed primary human hepatocytes to assess the inductive effects of bexarotene. Hepatocytes were treated with bexarotene 1 50M ; for 3 days. Docetaxel metabolism by intact cells and or microsomal fractions was then evaluated. Testosterone 6 -hydroxylation and CYP3A4- specific protein and mRNA levels were also measured to evaluate whether bexarotene acts as an inducer of CYP3A4. Results: Bexarotene, at clinically relevant concentrations 1-10M ; did not inhibit docetaxel metabolism. Also, incubation of primary human hepatocytes with bexarotene did not enhance docetaxel metabolism, which was consistent with the observation that the testosterone 6 -hydroxylation and CYP3A4 expression in bexarotene treated cells were not statistically different than those from untreated cells. Conclusions: Our findings suggest that bexarotene is unlikely to alter hepatic metabolism of docetaxel. Bexarotene appears to exhibit species specific differences in the induction of CYP3A and further studies are ongoing to understand the mechanistic basis of these differences and dolasetron.

3. Tolerance to Cold: A curious observation that is not emphasized in text books but noted here is that the patient dislikes heat and can stand cold very well. Even in the coldest days in Hongkong, the patient prefers to be naked and to be covered by a single cotton sheet only. The probable explanation is that the almost continuous muscular contractions and movements generate energy and heat in the body keeping the patient warm and dovonex.

To 24 months with respect to initial treatment corresponds to a higher percentage of therapeutic response and longer survival durations [5, 6]. The relative chemosensitivity of ovarian cancer has been used in the development of new anticancer agents. In the last five years, hexamethymelamine [7], paclitaxel [8], docetaxel [9] and topotecan [10], have been either approved in this indication, or reported to be active, after platinum-based treatment failure. Diaminocyclohexane platinum DACH ; compounds synthetized 20 years ago [11] seem to have a similar mechanism of cytotoxicity as first and second generation platinum compounds, but their markedly differential in vitro cytotoxic profile has recently led to the proposal of classifying them as a separate family of cytotoxic compounds [12]. Oxaliplatin is of particular interest, since it is the only DACH platinum currently in clinical phase I l - m.
